New Patients

Our practice collects the minimum amount of information needed to deliver quality care to you,  our patient.

In general, we require an accurate health history, demographic information, such as your legal name, address, and contact information, and any information needed to file insurance claims on your behalf (if you have dental insurance).

Please consider downloading, printing, and completing the forms below and bringing them to your inital visit. Alterniatively, please arrive at least 15 minutes before your scheduled appointment to allow time to complete them at our office.
